You will receive an email with a copy of your form. Make sure to let our salon staff know you have filled out your form online when you arrive at your appointment.
What to bring with you on the day:
- Bring your photo identification.
- If your child (under 16) is being pierced please bring your photo identification and Medicare card (or other proof of guardianship).
You do not need to print a copy of your form.
